Heartbreaking isn't it? Poor old Champ would be riding around on a
brand new ZX10R instead of his battered old gixxer thou. But it's
    worth it....

If the Senior Management are guilty, know they're guilty and are
    basically decent people:

"We have done some things exceptionally well in this project. However a
couple of oversights have blighted the overall result. A lessons learnt
session identified the structural and procedural weaknesses in the
project. Steps have been taken to address these with future projects
placing us in a far stronger position going forwards."

If the Senior Management are guilty, know they're guilty but are
    disfunctional human beings:

"A lessons learnt session and a study of the team, processes and
structure have identified the weak points. Remedial action will need to
be taken with the weakest team members, I suggest that we need to let
    XXX and YYY go"

Heh. I demand five-slide powerpoint presentations at least twice a
week. I then change a couple of words and slap my name on the front
    before passing it upwards.

See other response. What matters- learning from this or protecting
    yourself?

    Blimey, easily pleased, some people.

I'd not call it self awareness, well,
    from my observations, at least.
