I'm off to sunny Spain this year. As my E111 has become a little tatty
I decided I needed to get a new one. Good job too, as the system has changed
and will change again in Jan 2006.

Any E111 issued before Aug 2004 is no longer valid.

has further information and a link to the DoH website where you can
    download the forms.

Even if you are travelling fully insured you may need a valid E111 so
that you are exempt from paying the policy excess.

It asks for a personal identification number - not having needed an E111
before - which number do they want, is it National Health or National
    insurance or A N other?

    Either of the first two.

Also, you probably want to tick the box (just above the Declaration on
the CM1 form) for being automatically sent a European Health Insurance
Card, once they start being issued.
